4/28/2021,remote patient monitoring (RPM) alerts that the Patient’s blood pressure was elevated. Vitrics MA called the patient to address elevated blood pressure, she stated that she took medication at 7:30 AM. The patient states that she takes Metoprolol 25 mg in the evening and missed her dose last night but is feeling fine and not experiencing any symptoms. Patient stated she will wait another 45 minutes and will recheck blood pressure. Updated vitals were in accordance with the patient’s norm.
5/3/2021,remote patient monitoring (RPM) alerts that the Patient’s blood pressure was elevated. Vitric’s MA called the patient to address elevated blood pressure. The patient reported she is experiencing slight tingling in her hands, the patient usually gets tingling in hands when blood pressure is becoming elevated. Patient is having diagnostic testing from a mammogram recently performed and is a bit stressed. Dr. R had a telemedicine appointment with a patient and modified Lisinopril to 20 mg QD. Vitrics Nurse was notified. Vitrics Registered Nurse called and spoke to the patient regarding symptoms. Patient states tingling in hands began shortly after using her albuterol inhaler and had never associated these symptoms with the inhaler use in the past. Patient denied any palpitations, shoulder, chest or jaw pain . States that she feels safe at home and appreciates the monitoring service. We will notify Dr. R in the morning. Patient has asked that we notify her Cardiologist Dr P as well. A report was sent to the Cardiologist as well as notification sent to the patient’s PCP..

Vitric’s MA called the patient to address elevated blood pressure, and the patient reported she took her medications at 7:10 AM. Patient is not experiencing symptoms, and is currently making her breakfast. Dr. R notified. Vitric’s MA spoke to clinician for Dr. R, he is prescribing patient Hydrochlorothiazide. Dr. R would like the patient to follow up with cardiologist Dr. P, and for her to call clinic so she can have labs done in 1 week.
5/14/2021,Patient’s current trends have been reviewed. Patient is monitored for hypertension. Patient is showing signs of improvement but will remain tagged as the patient’s trends are not yet within goal.
